Step 1: Report
A University Facilities
Mechanic identifies a safety
concern or has a safety
complaint and determines that it
must be reported to the Safety
Committee.
The
Mechanic should:
Option 1: Complete the
on-line
Safety
Concern Reporting Form
Option 2: Print out the
printable version of the
Safety Concern Report Form
and send it to:
Mechanical Maintenance &
Planning Safety Committee
Facilities Operations & Services

Or Fax to:

MMPSC Responsibility:
1.
Your form is received
(electronically, via fax or mail
service) by the Mechanical
Maintenance & Planning Safety
Committee (MMPSC).
2.
The MMPSC will review and
discuss your concern in
committee
Step 2: Notification
The Area Manager is notified
that a safety concern exists
within the zone.
MMPSC
Responsibility:
After Committee review, the
MMPSC will forward your safety
concern to the appropriate Area
Operations Manager using the
Safety Concern Notification Form.
An abatement due date
will be indicated on the
Safety Concern Notification
From, providing the Area
Operations Manager with an
abatement time frame.
Area
Manager’s
Responsibility:
1.
Upon receipt of the
Safety Concern Notification
Form, the Area Operations
Manager will investigate the
unsafe situation, including if
necessary, conduct a site visit
for inspection.
2.
The Area Operations
Manager will develop an
abatement action plan to correct
the safety concern within by the
indicated due date.
Step 3:
Abatement
The Area Operations Manager
prepares an Abatement Action
Plan.
Area Operations Manager’s
Responsibility:
A completed abatement action
plan will be submitted
electronically by the Area
Operations Manager to the MMPSC
using the on-line
Safety Concern
Abatement Form
within the proscribed time
frame.
Should additional time be
required to correct the problem,
the Area Operations Manager must
request an extension.
Step 4: Resolution
The safety concern is resolved
and conclusions are communicated
to all parties
MMPSC Responsibility:
1.
The MMPSC will verify
that the safety concern has been
addressed
2.
The MMPSC will
communicate the action taken by
posting the information in
affected zone and on the
Safety Concern Resolution web
site.
3.
If the Mechanic chooses
to self identify, then the MMPSC
will communicate the abatement
action taken directly to the
Mechanic using the
Safety Concern Resolution Form,
sending the information to the
Mechanic’s
email.
